For any student interested in playing a sport, an Annual Athletic Pre-Participation Physical Examination Form must be completed and handed in to the school nurse. This form can be picked up at the school or found at https://www.state.nj.us/education/students/safety/health/records/athleticphysicalsform.pdf. It is advised that students receive a physical exam over the summer months to avoid inconvenience during the school year. A physical is valid for one year.

If a student does not have any interest in playing a sport, it is still recommended that students have a physical examination at least every two years. The exam must be conducted by the student’s own healthcare provider and the written report must be submitted to the school.
